Patient Requirements

Patients must live or work in Sevier County.

Patients must not have health insurance.

New patients need to bring:

• Proof of income
• Picture I.D.
• Social Security card
• Most recent tax return or other proof of total household income.

Noone is turned away. Regardless of your financial situation, you will be treated for your medical needs.
